Enrollment Process in 4 easy steps

Getting started with Momentum ABA LBA is simple and stress-free. Our enrollment process ensures a smooth transition for your child into therapy. First, connect with our intake coordinator to discuss your child’s needs and complete the necessary paperwork. Next, we’ll schedule a professional assessment to evaluate your child’s strengths and areas of need. Once the assessment is complete, we’ll handle insurance approval on your behalf, ensuring you receive the coverage required for services. Finally, therapy begins with a dedicated team of professionals working closely with you and your child to deliver personalized care and monitor progress every step of the way.

Insurance Intake


Connect with our intake coordinator to discuss your child’s needs and ensure they have the appropriate ASD diagnosis and evaluation. We’ll send you an enrollment form along with a few documents to complete and return to us to get started.

Schedule Initial Assessment


Once we receive and review your paperwork, we’ll schedule a professional assessment for your child.

Insurance Approval


Following the initial assessment, we will work with your insurance to secure approval for the necessary services and therapy.

Begin Therapy


An LBA/BCBA and behavior therapist will start working with you and your child, tailoring the approach to meet their specific needs and coordinate care with your child’s team thereby monitoring ongoing progress to ensure the best outcomes for your child.
